Your Breast History

If you have had a breast cancer diagnosis, BCT.org's Health History Form will ask you specific questions about your diagnosis such as the tumor's size, stage, and type; and any treatments you may have already received, including surgery, radiation therapy, chemotherapy, and hormone therapy.

Where to Find the Information You Need

  • Breast Cancer Treatment Plans
    Your Breast Cancer Treatment Plan/Summary or Breast Cancer Survivorship Plan will include all of the information you need to complete the Health History Form. If you have not received a Treatment Plan/Summary or Survivorship Plan, you may want to request one from your physician. The American Society of Clinical Oncology has developed forms doctors can easily use. You can view and download them here.
  • Ask Your Health Care Provider for Assistance
    You can print out the Health History Form and obtain the answers to questions you are unsure of from your health care provider
  • Medical Reports
    The medical reports from your tests, procedures, and treatments will include the information you need to complete the Health History Form. If you did not receive copies of any of these medical reports, you should request them, as you may need them at a later date. This will require completing a Medical Records Request/Release Form. Many medical centers have information on their websites about how to get their form (most have a PDF on their website), how to submit it, and the fees they charge to provide patients with this information.
